SECTION 1 – THE CUSTOMERS

The customers are at leats 18 years of age and legally capable of entering into a binding contract.

SECTION 2 – PRODUCTS OR SERVICES

Customers are invited to visit our web site at www.dentaldigitalcrown.com and submitted all your orders in  

Office Hours: 8:00am-5:00pm-Monday to Friday

Customers will receive an email confirmation the same day the order is placed as an order confirmation. Once the order is ready to ship, a shipping confirmation e-mail will be sent. 

Customers have to ensure that all information including the shipping address, part number(s), part quantities and type of shipping information are correct.

SECTION 3 – ORDER CORRECTION & CANCELLATION

It is the customer’s responsibility to contact Sam’s Dental Laboratory Inc. via phone (713-256-8869) and/or email (dentaldigitalcrown@gmail.com) informing Sam’s Dental Laboratory Inc. of order corrections.  Cancellation of orders must be made within the first 15 minutes that the order has been placed. Especially for STL orders. 

SECTION 4 – MODIFICATIONS TO THE PRICES AND SERVICE PRICE TERMS 

Prices are valid at the time an order is placed. We reserve the right to change prices without notice in any given circumstance. We reserve the right at any time to modify or discontinue the Service (or any part or content thereof) without notice at any time. 
The total cost of your order includes the cost of the products ordered plus delivery charges. 

SECTION 5 – PAYMENT TERMS 

Prior to shipment all orders must be paid in full. We accept  Visa, MasterCard, American Express and Discover. If your payment card issuer does not approve or refuse to pay for any reason, we shall not be liable for any delay or non-payment. 

All credit/debit card holders must be validated and approved by the card issuer. 

Purchases of our products will be marked as Sam’s dental Laboratory Inc  in your credit card statement. 

We are not responsible for any losses or costs incurred or sustained by the customer when transmitting credit or debit card information. 

SECTION 6 – SHIPPING TERMS

We will ship your product within 48 hours of regular business hours. (Monday through Friday- 8:00am to 5:00pm). 

The shipping carriers we use are UPS and FedEx.

 We will update your order with tracking information. 

SECTION 7 – REFUND AND WARRANTY TERMS

All of our cases are non-refundable.  However, your case can be exchanged within 15 days of purchase with a 50% remake fee. Your case will need to be returned back to Sam’s Dental laboratory Inc, not showing any heavy signs of wear or be modified in any way to receive credit towards your remake. A 60 day warranty applies to the fit and function of the appliance only.  Sam’s Dental laboratory Inc.  is not responsible for any incorrectly chosen tooth or gum color choices. 

 A custom appliance that is not fitting the way you expected, you can send it back to us for an adjustment or a redo. We are ready to make adjustments to your appliance at no charge.   In case of a second remake your appliance will no longer be eligible for a return.

We are not liable for any harm or damages related to the purchase or use of goods, services, resources, content, or any other transactions made in connection with any third-party websites. We reserve the right to refuse any order you place with us. It is our sole discretion that we may limit or cancel quantities purchased per person, per household or per order. 

These restrictions may include orders placed by or under the same customer account, the same credit, and/or orders that use the same billing and/or shipping address. 

In the event that we make a change to or cancel an order, we may attempt to notify you by contacting the e-mail, billing address, phone number provided at the time the order was made. We reserve the right to limit or prohibit orders that, to our sole judgment, appear to be placed by dealers, resellers or distributors.
